Acceptance of Terms

By using the Apex Specialty Referral platform, you agree to be bound by these Terms of Service.

Description of Service

Apex provides a referral coordination platform connecting general dental practices with specialty practices for the purpose of streamlining patient referrals, insurance verification, and appointment scheduling.

User Responsibilities

Referring providers must obtain verbal patient consent before submitting a referral through Apex, including consent for SMS communications about the referral. Submission of a referral constitutes an affirmation that the patient has provided this consent.

SMS Terms

By using the Apex platform, patients (via their dentist's consent) agree to receive SMS notifications regarding their referral. These notifications include appointment information, cost estimates, and scheduling links. Patients may opt out at any time by replying STOP. Reply HELP for assistance. Message and data rates may apply.

Limitation of Liability

Apex is provided "as is" for the purpose of coordinating specialty referrals. Apex does not provide medical advice or replace the clinical judgment of treating providers.

Modifications

These terms may be updated at any time. Continued use of the platform after changes constitutes acceptance of the updated terms.

Governing Law

These terms are governed by the laws of the State of California.
